Changing your contact details
To update details like your name, address, phone numbers or email address:
- fill out the change of contact details form below
- phone or visit the surgery, when we are open
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
You will need to show us proof of your new address by visiting the surgery with one of the following: driving licence, bank statement, council tax bill, mortgage statement or rental agreement or another form of proof of address. Alternatively, you can email copies of these to: enquiries.w92036@wales.nhs.uk
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your date of birth or name
You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
